Medicare has come out with a 5-Star Quality Rating System for nursing facilities effective December 18, 2008. Each nursing home will receive a "rating" depending on meeting specific criteria in areas such as:
  • results from surveys from the last three years
  • Staffing levels
  • Key Quality measures

As this report indicates, Medicare will rate each nursing facility from 1 to 5 stars, with 5 stars being the highest. Pleasant View Home is rated a "5 Star" facility.




Services Details

  • Skilled Nursing Services
    • Pleasant View Home provides skilled nursing care for those residents whose condition meets Medicare guidelines. A few of the guidelines include, but are not limited to:
      • Admission to an acute care setting for at least 3 consecutive midnights
      • Requires a skilled service that must be performed by a licensed nurse, Rehabilitation person such as Occupational Therapist, Physical Therapist, Speech Therapist and/or Respiratory Therapist
      • Skilled services must be ordered by a physician and that services must be provided at least 5 days a week. This information is substantiated by medical record documentation
      • Required TB test prior to admission
      • Required medical exam prior to admission
      • Required physician admit orders
  • If the beneficiary meets qualifying conditions, Medicare will pay 100% of the daily room rate plus covered ancillary services for the first 20 days. Beginning on day 21 through the 100th day, the beneficiary is required to pay a portion of the charges for these days for each benefit period. Supplement insurance often pays this amount. Call for current room rates.

    • 24/7 Professional nursing care
    • Personalized Care Plans to meet individual needs
    • Unit Dose medication for safe disbursal
    • Daily grooming and hygiene
    • Assistance with daily Activities of Daily Living (ADLs)
    • Certified Nursing Assistants
  • Restorative Program
    • Restorative plan of care is developed by the guidance from RN & Licensed Therapists
    • Instructions carried out by restorative aids and CNAs
    • Documentation completed to show progress of individuals
    • The activity dept. also has an exercise group daily Monday-Friday
  • Social Services
    • licensed Social Worker
    • Provided on a continuing basis to meet emotional, psycho-social and personal needs
    • Contact outside services when applicable
    • Living Wills, Durable Power of Attorney for Health Care, Durable Power of Attorney, and emergency issues of resuscitation are reviewed upon admission.
    • Completes a personal life history shortly after admission
    • Available to speak with resident or family member regarding questions that might arise.
  • Housekeeping
    • Clean resident rooms on a scheduled basis and as needed
    • Services provided on a daily basis
  • Laundry
    • The laundry department operates approximately 10 hours a day
    • Processes all washable personal laundry items and linens in-house
    • Services provided on a daily basis
  • Maintenance
    • Facility is completely equipped with fire sprinklers and an up-to-date fire alarm system
    • One part-time person keeps the facility in excellent working order.
    • Have a backup generator for operation in the case of an emergency
  • Business Office
    • Business manager has a business degree
